. Form DS-158  (all F-1, F-2, J-1, J-2, M-1 and M-2 visa applicants, regardless of age, are required to complete form DS-158)
. Form DS-160  (required at only a few selected U.S. Embassies/Consulates and is not currently in full implementation. Where in use, the DS-160 replaces both the DS-156 and DS-157 and is an integrated online application form. In order to apply for a nonimmigrant visa, all applicants must complete a Form DS-156. All male nonimmigrant visa applicants between the ages of 16 and 45, regardless of nationality, will be required to complete Form DS-157 in addition to the usual DS-156 Nonimmigrant Visa Application. In addition, all male or female nonimmigrant visa applicants holding Chinese, Cuban, Iranian, Iraqi, Libyan, Russian, Somali, Sudanese or Vietnamese passports may also be required to fill out the DS-157. Please keep in mind that any Consular Officer can require any visa applicant to complete and submit a Form DS-157. In addition to these two forms, all applicants for F-1, F-2, J-1, J-2, M-1 and M-2 visas, regardless of age, are required to complete form DS-158.
